Bottom bun (toasted), tomato sauce and/or barbeque sauce, meat patty, bacon (crisped), cheese (melted), red onion (lightly fried), lettuce, mayo, top bun (also toasted, seeds optional). If i'm really hungry, and the resources are avaliable, i'll have an extra layer of meat, bacon and cheese.
I take my burgers very seriously. The best are usually homemade or from fish & chips shops. While I do like the McDonalds burgers, they are in a different taste group to those burgers, and I can't honestly say which style I like.

Two white bread buns, with two medium done pieces of beef. Put two slices of sharp chedder cheese inbetween. Pit 3-5 slices of pickle, sliced olive, sliced onions, and possibly potato chips, depending on my mood. All on the bottom. On the top, use tjai sweet chili sauce, pickle relish, musturd, a dash of BBQ,and extremely hot hot sauce. Serve with fries. Do not use ketchup on anything, it ruins it.

I make my own spicy bean burgers (potato, egg white, red onion, kidney beans, butter beans mashed up, peas, sweetcorn, spices, breadcrumbs, etc..). Get a nice thick wholemeal seeded bun, butter it up, layer it with various kinds of cheese, stick it cheese-side-up under the grill for a minute or so while my burger finishes frying, slap the bad boy together, add mayo, devour.

Can't eat beef.
I stick to chicken sandwiches, which are good. Regular sesame seed bun, preferably toasted on the bottom, lettuce, tomatoes, mayo, and ketchup. Oh, and some fries.

1" lard-fried beef patties on the outside instead of bread, containing a grilled cheddar-covered sphere of bacon surrounding a block of cookie dough ice cream, and splashed with a shot and a half of Kettle One vodka. Preferred method of consumption involves surgically lining the consumer's arteries with the burger.
On a slightly more serious note, anyone vetoing ketchup has clearly never enjoyed a well-mixed sauce made from mayonnaise and ketchup on his burger, and is therefore an uncultured ignoramus. That pink sauce is delicious.

On burgers? No thanks. With chips? Yes please.
If I'm going to mix I much prefer ketchup and sweet mustard.

Bun, burger, cheese, cheese, ham, lettuce, carrot, alfalfa, tomato sauce, tuna, top of bun. My home-made burgers are basically a giant salad sandwich with a burger shoved underneath.
