I find myself switching between options 2 and 3 for informal writing, though recently I've been sticking to the former. I never use the last version (in fact I had to look at my keyboard to remember how it looked), and I would hope no one uses "+" at all. You can vote twice, mostly because I'm inconsistent and never use just one.

I do a reverse version of #2.
Sort of a lowercase epsilon with a line through it.

I use 4 in rare cases, usually when I'm in a great hurry.
Most of the time I just write it as it is.

Formally, spell it out unless it needs an ampersand in which case I do the ampersand. Informally, often spell it, but otherwise I do a little ribbon:

I only use an &pers& when I'm writing it as an operator, e.g. bitwise or logical AND.

When I have more time, I draw a little guy flying a kite.

1 and possibly 3. 1 is like 99.9 % of the time.

Actually, I've never used anything other than 1.

I'm pretty sure that my handwriting makes it faster to write 'and' than '+'. Anyone else?

The wacky Portuguese ampersand looks too much like the lowercase 'L' I write when I use it as a variable.
