Today is Metanet's National Username Change Day. If you would like to change your username from its current form to any new username, please download the attached form, fill it out completely, and attach it to a new post here. This is a one-time change, and all changes will occur tomorrow, June 24th at Midnight PST. Please only post once in this topic, and only fill out the attached Username Change Request form*. Any other requests will be ignored, and any further posts will result in disqualification of your application. Please fill out the Username Change Request form completely, and attach it to your post. After this 24-hour span is up, no other username changes will be seen.

If you have any questions about the Username Change Request Form, please post here.

Instructions:

Request No.: Request number is the number 0411, followed by your post number, followed by your member number here on the forums. Post number can be found by posting in this topic, and linking to the post. Member number can be found by looking at your member information page.

Request Date: This is the date of your submitted request. Failure to fill out the correct date will result in rejection of your application.

Request Title: This refers to the title of your request. Requests should all be entitled "Metanet Username Change Request Contest, Vol. 2". Failure to title your request correctly will result in rejection of your application.

Status: Your status will begin as "Pending" As the request processes over the next 24 hours, you will be required to update your application as your status changes. The statuses are "Pending", "Processing", "Accepted", "Post-Processing", "Green-Lighted", "Finalized", and "Post-Accepted Green Light Boogie Time". Failure to update your status correctly and in a timely manner will result in rejection of your application.

Originator's Name: This refers to the name of the person who referred you to Metanet. If you were referred from NUMA circa 2004-2007, please put "Arachnid," or "Care of Google, Inc." If you were referred by another member, please list that member's username, Member ID, and dates active here. If you were referred by the game's built-in software help files, please put either "M&R," "R&M," or "Raigan and the Horse-Woman." Failure to follow these guidelines will result in rejection of your application.

Phone/E-Mail/Mailstop: This field is for office use only. Please do not place anything in this field. Failure to comply with this will result in rejection of your application.

Sponsor's Name: Please list the name of your Metanet Forums' Chevy/Motorola Sponsorifix® ForumGuide. If you do not have a Metanet Forums' Chevy/Motorola Sponsorifix® ForumGuide, please put "Not Applicable" or "N/A". Not having a Metanet Forums' Chevy/Motorola Sponsorifix® ForumGuide, your turnaround time will be increased from 24 hours to 4-6 weeks. Failure to list your Metanet Forums' Chevy/Motorola Sponsorifix® ForumGuide's name will result in rejection of your application.

Priority: For all normal requests, please list "Standard" as your Priority. If you wish for your request to be expedited, please list "Urgent" as your Priority. If you will be away from the forums for an extended period of time during the time your request will be processed, please list "Delayed" along with the precise date when you wish for your application to be processed.

Assigned To: Please list any and all websites with which your username is assigned to. If you are registered on other websites under the same username, we will need to know any and all of those sites. We will manually verify your ownership of each account listed. Justification: Please list any additional reasons for your request here. We require at least three forms of justification for this request. Failure to provide at least three forms of justification will result in rejection of your application.

Alternative Solutions: This field is for office use only. Please do not place anything in this field. Failure to comply with this will result in rejection of your application.

Please leave Page 2 Blank until your application is in "Accepted" status. Once your Application is "Accepted", you will be required to fill out page 2 of your application completely within fifteen minutes of the response date's ending time (11:59:59), or your application will thusly be rejected.

I'm sorry, but I was not able to contact the phone number listed, and when I tried to send a query to the e-mail address listed, Gmail returned a subsystem delivery failure. Is there any other way I can contact someone about this? Preferred methods are PM and IRC.

After a quick quality check of your form, I have come to the conclusion that the number of fields within the form are below the MFAS-2008 standards, therefore the form is not valid and will not recieve the approval Metanet Form Assocciation. This is a severe problem and should be corrected immediatly by increasing the amounts of fields. This increase must have been completed by June 24th 11:59 PST, if these improvements are not completed, then you will have to PM the CEO of the Metanet Form Assocciation.
