Today is Metanet's National Username Change Day. If you would like to change your username from its current form to any new username, please download the attached form, fill it out completely, and attach it to a new post here. This is a one-time change, and all changes will occur tomorrow, June 24th at Midnight PST. Please only post once in this topic, and only fill out the attached Username Change Request form*. Any other requests will be ignored, and any further posts will result in disqualification of your application. Please fill out the Username Change Request form completely, and attach it to your post. After this 24-hour span is up, no other username changes will be seen.

I'm sorry, but I was not able to contact the phone number listed, and when I tried to send a query to the e-mail address listed, Gmail returned a subsystem delivery failure. Is there any other way I can contact someone about this? Preferred methods are PM and IRC.

After a quick quality check of your form, I have come to the conclusion that the number of fields within the form are below the MFAS-2008 standards, therefore the form is not valid and will not recieve the approval Metanet Form Assocciation. This is a severe problem and should be corrected immediatly by increasing the amounts of fields. This increase must have been completed by June 24th 11:59 PST, if these improvements are not completed, then you will have to PM the CEO of the Metanet Form Assocciation.
