How to treat an ugly person?

Ok, now don't get me wrong. I don't have anything against her. I just thought, what if I ran into her in real life. What could I say to her without feeling bad because I could:
1) Be truthful and tell her she is ugly since she's fat.
2) Be nice and tell her she is good at what she is doing, but then I'd be a jerk for being a liar.
3) Just not talk to her, but then I'd be a jerk again for ignoring her. (since I'm sure she is doing this type of thing for attention)
Any opinions?

I am not exaggerating when I say that left me winded. She is ridiculously, profoundly ugly. I think this may very well be the most hideous person I have ever seen, and this is coming from a person who has, in his past, actively sought out pictures of hideous people to add to a collection of the same. If I were alone with it in a room, I'd stay on the opposite side of the room and hit it if it came close.
I might've said something different if she actually had any singing talent, but she very doesn't. She sings about as well as any other random child with no formal training or talent, and the fact that she chose to sing a Miley Cyrus song leads me to enough assumptions about the sort of person she is and will probably be that I don't think it's a shot in the dark to say she's not going to have very many redeeming features. What we're looking at here is the factor that balances out all the supermodels in the world to set the average for attractiveness that most of us know in our day to day. This is the extreme left of the bell curve. I don't know who let it out of the basement and gave it a camera, but I suppose it's an important reminder of the horrors in the world that exist apart from television and other popular media.
If someone I knew was this ugly, I would take pains to distance myself from it. I wouldn't be up in its face about it, and I definitely wouldn't go out of my way to make its life less pleasant (it probably suffers enough of that already), but I'm sorry, I'm not a fucking charity.
And before you get on my back about judging people for something they have no control over -- I hold firmly the belief that people are only ugly by choice. Some people just aren't blessed with good looks, but that only makes them unremarkable, not ugly. Take a look at her face -- if she lost weight, tanned a bit (just something more than her current ghostly paleness), and... actually, she could probably keep her hair the way it is now, she might even be attractive. She'd probably have to go with a tomboyish look what with that upturned nose, but if you add a touch of dark/gothy makeup, she could even be on the cute side.
Matter of fact, her facial structure reminds me of a girl I know. A totally dispassionate look at her would show you that her head and face are actually more than a little deformed. Her forehead is, like, half of her head, her lower jaw is tiny, and her eyes are kinda bugged out, low on her face, and far apart. But she's fit and very athletic, and she knows how to style her hair and apply makeup, to the point that she's actually one of the more attractive people I know (in this small town, anyway). She gets looks from guys all the time. I've seen guys compete over her.
But this lagoon creature? I have no meaningful amount of sympathy. She did that to herself. She stuffed those ho-ho's in her face. She's the one refusing to put effort into her appearance. I don't respect people who don't respect themselves.
