Go to 'Account' in the top right corner.
Click Application Settings.
Remove application.
If you really really really must have the app, you can reinstall it, and your friends will no longer be incorperated in it. Bear in mind however, your crops/mafia gang/fish/bears/lesbian jugglers/whatever the fuck you play now on facebook will all be gone.

Yeah, removing the app then reinstalling it will remove all saved data on it's database for you, including friends related to it that you have chosen to add.
