Any rape scenes always get a reaction. Usually it is disgust, but in the case of the one in A Clockwork Orange, the fact that he was singing "I'm singing in the rain" made it all the more inane and traumatising.

Top of the list, Requiem for a Dream. In particular the very end with the quick scene transitions. It always leaves me wide-eyed and shocked like I've just been slapped around the face.
Vanilla Sky left me feeling pretty spaced out the first time I saw it. The scene in the bar when the room goes quiet always gives me goosebumps and the film in general just really makes me think. Similar with America Beauty too.
American history X, very shocking film.
Eternal Sunshine of the Spotless Mind is another good one to mess with your head.

I've jumped numerous times while watching movies...Spiderman 3 had a pretty surprising part.
There was also a Jet Li movie where someone poisoned his tea so he would lose to his rival guy, and he ended up puking all over him. That grossed me out pretty bad, emetophobe-hypochondriac that I am.

I forgot about Happiness. Quite possibly the most disturbing movie ever produced.
I thought the happening was pretty good. It did help that we were having really weird weather when we went to watch it. Hale in June in Buffalo?
