If the exploding toilet made a hole in the ceiling, try to get up in there (e.g. by using the bed as a step or something).

It's kinda damp and looks weak, but there's no hole.

Anyway. Take the gun and wake the guy up, if he isn't already (I assume he is, as he hasn't pointed the gun at you). Try and get him to tell you more about where you are. If he retaliates, kill him. If he won't give you answer, shoot him in the leg. If still won't give you answers, kill him.

Anyway, easy solution is to remove all bullets and when confronting enemies, point it at your own face and pull the trigger repeatedly. The decapitation effect would go forwards then, right?
